Outreach initiatives and extensive joint working led by our innovative Health Trainer coordination team. Our team develop relationships with community groups to streamline patient pathways, including hard-to-reach groups to ensure that they are able to access services and health information whilst overcoming barriers to communication.
Promotion of learning throughout the community including work with students at Southbank University, GPs and other practitioners. We place high value on engaging patients in service provision and redesign, providing support and development to our staff and transferring knowledge - not just records - to local GPs and healthcare providers.
