Evolution Health has been selected as the 'provider of choice' for schemes under the 'Equitable Access Programme' as tendered by Greenwich PCT. Our first project will be to establish a new GP practice in the Charlton and Kidbrooke area which will be followed by an additional scheme in late 2009/early 2010.

What is our focus?
We will be focused on providing:
1. High quality patient care at all times
2. Improved access for registered and unregistered patients
3. Improving primary prevention with existing patients and with outreach work with hard to reach and
4. Good control of chronic disease
5. Integration with the local health economy, other primary health care providers and community groups to improve health outcomes
6. Community outreach initiatives using an Outreach Coordinator to develop this programme.
7. A mobile unit to deliver healthcare and outreach initiatives to the hard-to-reach population and in convenient locations for local residents
8. A working environment with a focus on staff training and development
9. Training accreditation to enable us to train GPs and other clinical staff
10. Standards based on the Royal College of General Practitioners Quality Practice Award which we aim to achieve within a few years.
