To register with an Evolution Health practice you must live within the catchment area.

To register at Thamesmead NHS Health Centre you will need to live in the London Borough of Greenwich. If you don’t want to register with us, you can still walk in and be seen by a member of our clinical team.

To register at Shooters Hill Medical Centre you will need to live in the Shooters Hill, Charlton or Kidbrooke area. You can check with reception on 020 8331 3400 to see if you are within the catchment area.
Simply drop into the centre and complete a registration form at reception or you can download a registration form using the link below. Once completed, a member of our team will check it and ensure we have all the information we need. We will require some evidence of your address - this can be a recent bill or letter with your name and address on - along with some evidence of your identity. You will also need to book an appointment for a new patient health check.
! Please Note : Registration forms can be filled out in advance for both centres by downloading the form below or by applying online. However, the process is not complete until the documents have been signed in the surgery and a health check has been carried out.
