We are currently accepting new patient registrations at Shooters Hill Medical Centre. If you or a member of your family would like to register then please download and print the appropriate form below.
Once completed, please bring it along to our surgery where a member of our team will be able to check this and ensure we have all the information we need. To ensure that we are registering patients who live within our practice area/Greenwich we will require some evidence of your address - this can be a recent bill or letter with your name and address on, along with some evidence of your identity. Please ask if you are not sure if you are living within our area. You will also be required to fill in an NHS form at the surgery but the forms above include the majority of information we will require.
